The wine represents his blood which was shed on the cross. It is his blood that cleanses us of our sins. For example, instead of taking wine , grape juice is a wonderful substitute. In fact I buy a six pack of grape juice, in that way it lasts a long time. In general, any liquid would work, apple juice, water or even milk, The words you speak combined with the elements you have at home will be powerful.

If you are asking the question, “can communion be taken at home? Jesus took communion with his disciples and it wasn’t in the temple. The last supper took place in the upper room, most likely a room on the second floor, and perhaps on someone’s roof in Jerusalem where the disciples were staying. Have the leader or another participant to close in prayer. Or, as a group, recite the Lord’s prayer to close communion at home.
You don’t have to have a group, an audience, a priest, or a pastor. You can remember Jesus in this special way that He prescribed, at home. The wine or fruit of the vine symbolizes the blood of Jesus. The blood of Jesus washes all of us of our sin. There is a big difference between how we celebrate communion and how the early church celebrated it.

In the early church, communion was more of a full-fledged meal instead of a small wafer and a little thimble of grape juice or wine. They would celebrate the feast together as a community and would sit down together.
Likewise you don’t need to use unleavened bread. However when I’m taking communion I prefer the inexpensive Matzo crackers for taking communion.
